Create a Lightview Form & Extension for a Joomla 2.5 Site

Seeking an advance Joomla 2.5 programmer to create a lightview for a form and to showcase two joomla components within the lightview. See attached PDF file for a visual flowchart. Everything displays in the lightview. 1. There is an initial form 2. Depending on users selection from form they are directed to another form 3. Depending on selection from second form a slideshow will play. This slideshow is a component of joomla. 4. After slideshow plays content is available for the user to view. The content is base on the selection made from second form. The content is within the same category as the slideshow. 5. There is another form and content. 6. There is a quiz and the quiz is a component of joomla. 7. There is some content. I also need an custom error page added so my server provider's error page does not display.
