Basic PHP Coding Required.

This is what we need: We need a webpage with a large form field in the centre of the page for the customer to enter their email address. When they have entered there email the should press button that says "Go". This should then redirect to an affiliate link of ours which we decide: For example: [login to view URL] But what we want to do is automatically add the email address that was typed in the form field to the end of the link For example: [login to view URL] The design doesnt need to be brilliant just needs to be in a box in the centre of the page. If you can do this quickly then bid away :)
