Alpha & Beta Test Reporting Website

We need a small website consisting of a home page where we will be sending users of our mobile app to report beta testing results. Once on the site, the would log in with their user name and password and then be taken to a screen where they can fill out a survey type form of about ten fields or check off boxes rating certain features and functions and make comments. The form data would be held in a database where they can view previous reports made and edit them as well as the admin being able to search reports and to status check them as completed. Each new report when submitted would result in an email being sent to three addresses, the user, our programming team and our admin. Graphics will be minimal on the site. We would also like a blog where we can post information and updates for the users. Blog and history as well as "report a new test" would be menu selections after entering in user name and password. We want something simple, inexpensive and that we can get done quickly. Would also like the ability to change field names or to add up to five additional fields ourselves if we find we need them. This website would be added into our domain as a [login to view URL] subdomain. Bid must include deployment onto our server when completed for final portion of payment and all source code and a completed copy upload-able via FTP. Finally, this site must be able to recognize mobile users and format to a mobile-friendly interface. Please read the project first and write the word "Understood".
