Create a package installation of Ubuntu + My Application

We have a simple PHP/MYSQL applicaiton and we need to package it into a Ubuntu 11.10 ISO. I have tried to follow the JEOS guides but not having luck. The install CD must setup ubuntu, install the packages, then on first login ask some questions such as ip, dns, 2 specific questions to be added to a file.. and thats it. We have a scripted bash script on how to setup the rest.. such as php,mysql, some basic users we need, git, etc... so your job is half done. You must document the entire process.
