Add a ''Sign up for newsletter'' checkbox to donation form - Mailchimp Integration

A short and simple job for the person who knows how to do it: I need to integrate a checkbox for a newsletter on a donation form with a MailChimp list. The users name and email should be fed into the MailChimp account if they check the box. The API for MailChimp can be found here: [login to view URL] You will do the code in the attached file, and I will then replace your test variables with our real API number and LIST ID etc.
