FORM INPUT Checkboxes with sound number and name. (current sound selection page) Ability for us to add, subtract, or rearrange the sounds from the back end code. Captcha check with submission. Confirmation “thank you” page. Minimum Data Points 1. First Name 2. Last Name 3. Invoice # 4. Phone # 5. 350+ Sounds Bonus Create a floating counter that lets the user know how many boxes are checked. OUTPUT Responses emailed to us. The selections should be in the order selected. Only checked sounds will show up. Vertical orientation top to bottom. Columns would be nice as data fills up. The selections should fit on one page, max two pages. We should be able to print the page with ease. Bonus Selections show up left justified. As sounds fill up page, they are organized in columns from left to right.
