I need some graphic design. Hi I'm launching a Health MOT business alongside my Personal Training business. What I need is a template designed using a traffic light system on each test I perform. Red / amber / green Top of the page will contain my business logo and Health MOT Plus. with the slogan - improved health = higher performance (Space for answers below - no traffic light for this section) Name M / F Age Height Weight Blood Pressure (with space for results) systolic diastolic Pulse, again with space Irregular pulse Y/N Blood Glucose with space for result Type 2 Diabetes risk Y / N Blood lipid profile: (spaces for results below) Total Cholesterol HDL Cholesterol (good) LDL Cholesterol (bad) Triglycerides (blood fats) COPD: (space for results as per) FEV1 score and ratio FEV6 score and ratio Lunge age Peak flow Body composition (space for results in lbs) Body fat percentage Lean tissue mass Fat mass Hip / waist ratio BMI Hand grip strength (space for results in lbs) Left hand Right hand Postural assessment : (extended space for writing notes) Further treatment recommended: (extended space for writing notes) Re-assessment date: Please note, each section needs a traffic light system on the right so I can score their sections in simple terms, Any questions please ask.
