Hi: I can design a form in excel with check boxes and drop down, however,I have a clarification: How do you want to use the stored information? How do you want to access the information after it has been stored. Do you want a menu at the beginning of the sheet asking you to choose whether you want to enter the data or view the data. On choosing a appropriate option you will be directed to a specific sheet - If you chose data entry, you will go to a sheet that will contain the form. You can enter the data and click a button to store it - If you choose data viewing page, it will take you to a page where you can view the data. If you want, I can add a edit/ delete option allowing you to edit or remove previous data If you award me the project I will design a sample form and send it to you for your review. this is what i can do really faster, have done around 200+ project like this. there might not be any maths calculation but your user form might need lot of validation to accept the valid data, isnt it ? please send me instructions, i can revised my bid if there are less work of what i am thinking.
