Data Entry - Look image and fill a form based on it.

We have around 400,000 hockey cards and buying around 10,000 cards per months right now. We have a fast service that can scan the cards but we need someone that will go to our web page look at the card image then fill the small form on the right of the image based on what information found in the image. I provide you a prototype screenshot in attachment to help you figure out what it look like. Everything will be web based. There is text box that need manual typing and others are predefined drop down list to pick item in the list that correspond to the image. This is a very easy task but it's repetitive. At first we will test you with around 2500 cards and after that if everything goes well and your still interested we may have around 5000 cards to do each week and maybe more. What we need you to give us your best price based on 2500 cards. IMPORTANT!!!! I will then divide this price by 2500 to fix a price per card for all the new contract. So just give me your best price for the 2500 card and don't forget that this price will be the base price by card for the next batch of cards.
