Tidy Up HTML Optin Form

I have an HTML "optin form" with aWeber name and email capture fields that I need help finishing.

I would like you to make it look more professional.

To see what? I have so far go to:

I'd like the? form to look something? like this one;

I would like my form to be smaller by 25% if possible by shortening the height.

I would also like mine to have a uniform background color in a light to medium blue.

1. All the HTML as a Notepad document.

2. The Image (It's an Image I purchased from bigstockphotos)

3. A text file that contains my 2 bullet points that describe my reports contents.
