We are looking to create JavaScript form that generates an insurance quote. It will have a 1 input, and one output field which would be a price quote.

Here is a sample we found.

We will be embedding the script on our Word Press site, therefore it must work with Word Press.

It will look like this:

Estimated Yearly Payroll: [ Input ]

[Submit]

Estimated Premium: [Output]

Formula is basic.

For any input between $0-30000 = $950

30001-50000 = 1150

50001-70000 = 1675

70001-90000 = 2175

90001-110000 = 2550

110001-150000 = 2995
