Develop a guaranteed way to load a JS file and set SRC attribute at any point (Lazy Loading)

I need to be able to add a ScriptElement to the Head of an HTML document and ensure that it is added to the page. Needs to work on all browsers and needs to work in all states.

This needs to be done is straight JavaScript. No libraries (ex: jQuery, MooTools, etc. can be used).

If you are not aware of the technical concerns this bid proposes, do not bid.

The attached .zip file works to an extent, but what I need delivered is a solution that works in all situations. You will need to review and understand the limitations and what needs to be done to make it work on all browsers and in all conditions.
