Hi i need some data entry done on approx 14,000 records from a physical 304 page book that i will snail mail post over. So please only place a bid if you have a physical address i can send the book to.

I have attached some pictures of the pages of the book to give you an idea and also attaching the format i want the data to be in.
i will need the data for each item in this book entered into a sql server database or mysql or csv file or excel spreadsheet in the specified column format i supplied in the csv attached
The records will be checked and if there is to many records that are entered in error then payment will be reduced or withheld. I expect a 98% correct record rate.
On commencement of work i will require just 2 pages to be entered and emailed to me to check the formatting before continuing.
