Data Entry into word

I have 4000-10000 names and address printed out that need to be entered into label format in word. The info provided is clearly printed out and easy to read. I just need them enter into the label format in word.
Please provide a quote for 4000 entries and a quote for per every 1,000 above that.
I need a consistent amount of work done every day. I need at least 50 names entered on the first day of work. I am flexible on how many names are entered each day, but I need a schedule on how many will be done each day.
I will provide at LEAST a 25% bonus if the work is done in a high quality fashion with little to no errors. If communication is consistent/timely and if the schedule is meet. Schedule will be decided when the coder is selected and based on the coder capacity.
Deliverable are word files with the names and addresses entered.
I will provide the label format needed.
The source data will be in adobe acrobate image format. I will need the data entered into an excel spreadsheet one entry per row first, then use Word Auto Label Generator togenerate the doc file. I will need both files. I will also need instructions on how to use word auto label generator.

## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
