Script to create config file from CSV entries

I have config files for phone extension buttons that look like:

[login to view URL] = 16

[login to view URL] = 1

[login to view URL] = 835

expansion_module.1.key.1.pickup_value = **

[login to view URL] = Alan

[login to view URL] = 16

[login to view URL] = 1

[login to view URL] = 808

expansion_module.1.key.2.pickup_value = **

[login to view URL] = Barry

[login to view URL] = 16

[login to view URL] = 1

[login to view URL] = 806

expansion_module.1.key.3.pickup_value = **

[login to view URL] = Colin

I need a script created that will take a csv with value and label fields like below and turn it into the config above

835,Alan

808,Barry

806,Colin

I would prefer this to be a Powershell or VB script.
