i need a mysql db to hold a list of represntatives. i need to a form that will look them up by zip code and send an email to the matching represenative.
To further clairify:
Summary: I need a form that will preform 2 main functions.
1. Do a lookup to find a representative based the data entered in the form's fields.
2. Send an email to this representative and store everyone who sends/submits an email to their representative found in step one.
I need a form that people will enter their first name, last name, and town (maybe state).
Town should autocomplete field based on the the list of our available towns for purposes of exact match.
Upon submit we will look up and display the representative associated with the town
User will then have an options to press submit to send an email message to the representative. The email should include the ability to merge the person's information i.e. fname,lname,town.
We will also log each user. I will need to be able to retrieve the user file in csv format.
It should support the ability to create urls with variables that will autocomplete the form (step 1) to find the representative if clicked. For example: http://[domain]/fname+lname+town+state
Upon successful submission resulting page asks some yes/no toggle qustions, asks for donation and links to our paypal.
Please estimate and advise on development tools that you use and environment required.
