Online booking form for birthday parties

I need a simple online booking form to book birthday parties. The application must be done as single page app using Angular 1.5 (not 2), php , Bootstrap and MySQL as my website is hosted on a Linux server.
The application workflow is very simple:
1. User fill in the details (Name, Phone number, Email, Birthday Child's Name, How old they will be turning, approximately how many people will be coming)
2. Step 2 will be a display additional items items they can purchase it for the party (i.e. balloons, hot food,candles etc). This is user maintainable to so will need an interface for admin to maintain it.
3. A nice looking responsive email template (1- 2) (please look at litmus)so they can prefill this and print it out or email it to party guest. And also a list for them to fill out their guest list of people they will be paying for on the day. This part would be optional as most people wont know who they are paying for at this stage.
4. For reporting I will need a list of parties that are booked for the day with the following details: Date time, number of people booked and what they purchased and whom they have paid [login to view URL] the person comes to pay for the party at the venue admin should be able to see the party details including age groups (5-7 year olds,10-15 etc)
This is basically it however there will be few more extensions later on.
