Need a PHP form to post to a .net Web Service and return a confirmation.

I am in need of a small contact PHP form for our WordPress site. The form collects name, email and zip code. It needs to pass this info to a .net web service using SOAP and then continue to a confirmation page based on what the SOAP service sends back.
Basically the the form submits to the service and it checks the zip code to see if we have products in that area then it either sends a "NO_ERROR" or a INVALID_STATE" if no products exist.
If you can write the code I can add it to Wordpress, as it will be used as a simple lead form. Also it would be good if it used AJAX to keep from refreshing the form on confirmation.

Use the function called wsInsertLeadFindNearestDM
There are a lot of fields to use but just send firstname, lastname, zip and email.
If the submit is successful the return will say NO_ERROR
If we do not have product in the state the return will say INVALID_STATE
Then, after passing form data to web service... and based on the return value from zip code. I just need a Thank you message or a sorry we do not offer insurance in your area.
