Building a local website to run reverse image search(need this done ASAP)

I need a simple 1-2 page website. On one page a user can upload an image to see if it listed anywhere on the internet(reverse image search, google/bing/whatever) and on the second page display the results if it is. Afterwards I need a SQL database added to this project that will keep track of all the images uploaded and their results.
The first part HAS to be done today by 5pm EST May 3rd. The database part can be postponed by a few days if needed.
I will pick someone within 30 minutes of the posting so they will have roughly 3.5 hours to deliver on the first part, please don't bid if you don't think you can complete in that time frame.
