Website Design: Banner + CSS Templates

My website needs better CSS templates than the current, which are based on an open-source template.
In terms of graphics design in Photoshop the following is needed:
A) Design a pleasing banner with menu. The banner must have a small logo inserted. There should be two versions of the page banner:
1) menu only (with logo inserted) (about 8% of page height)
2) full banner - menu plus graphical image (about 25% of page height)
B) Design the page border in a theme consistent with the banner design.
In summary, you need to Photoshop one thin-border page in two versions: with a tall banner and with a short banner.
Please use 1280 x1024px as standard.
The website has nine pages, based on five style sheets. The styles are:
1) logon screen [one page]
2) main menu screen with 10 buttons [one page]
3) a two-frame screen with active content (ASP) [two pages]
4) a simple information page [three pages]
5) a single frame tabular display of active content (ASP) [two pages].
As a proof of your design you will need to load the content from the current website, one page per style (5 pages)
I will provide the current templates to the winning bidder as a starting point.
You are welcome to use your existing template or a stock template and just modify it for this content.
PLEASE SUPPLY SOME EXAMPLES OF PREVIOUS STYLES - DEVELOPED OR IMPLEMENTED FROM STOCK!
Please note:
I am looking for a person with Dreamweaver skills and graphical design talent. This project does not require any coding, only Photoshop and Dreamweaver. It is a fairly simple design. There are two types of pages:
1) "working pages" - consist of two frames populated from my ASP code; these pages require no layout - the maximum screen realestate is dedicated to windows coming from ASP, so the header menu needs to be minimised. You need to design only a thin header menu and a matching screen border.
2) "information pages" - simple pages, such as Logon, About and Contacts; these pages need to have a nice banner below the thin header menu.
