Looking for a freelancer to setup a simple survey with e-mail submit in HTML.
Survey later will be used as an iFrame in Facebook Pages (which won't require installation on your end).
How it will work:
1. Person goes on Facebook page, and clicks on the "like button"
2. Person gets a survey (which is inside an iFrame through Facebook)
3. Fills in 10 answers
4. Clicks "SUBMIT"
5. Questions/Answers are emailed to a certain email
6. Popup within iFrame says "Thank you for submitting, please enter your full email and address to get the product".
7. Person enters their email and address and clicks, "SUBMIT"
8. Address/Email is sent within the same email as the questions/answers
9. A "Thank you" appears, waits 3 seconds and redirects them back to the Facebook wall.
10. IF the person is already liking the page, and clicks on the tab, it will redirect them back to the wall (so they can't resubmit their info over and over again).
Questions for survey, and graphics will be provided to the person who wins the project.
