Convert XFA PDF Forms to HTML 5

I have multiple XFA PDF forms to be converted into HTML5 or another format that will work on Windows, macOS, iOS, and Android. Most of the forms share scripting. The forms range from three pages to 12 pages. The forms much exactly match the original forms in both appearance and functionality.
The first form to be converted will have:
Cover page
Four pages with a variety of form fields including date fields, drop-down selections, and basic text fields.
An expandable Photo Section with options to place three photos on a row, two photos on a row, and a single photo on a page.
You are bidding on doing only the first form. After the first form is successfully completed, you will have the opportunity to create other forms. I need to have approximately 10 forms completed by mid-December.
Please submit an example of an XFA PDF that you have converted to an HTML5 or other cross-platform format. Your example MUST include dynamic elements such as buttons to add rows or sections and buttons to insert photos.
Please DO NOT submit links to websites. Only submit examples dynamic forms. If the example is on a website, submit a link DIRECTLY to the form. Offers that do not include examples of forms you have created will NOT be considered.
