Require graphic artist(s) to design my forms (see sample attached) into Illustrator CS format forms (A4 size).

I have about 30 pages (not 30 sets) of them. Basically, it's simple job but need to take alot of time todo this. I have done some. Please express your bid and I will email you more samples I've done in Illustrator format, so you get a clear idea.
