Use jQuery to Modify UI of SP Visual Web Part

We have a Visual web part that contains an update panel... inside the update panel is a listview that uses both the header and item templates.
Inside the list view in both the header template and the item template are several controls. There is a asp.net checkbox called isCurrent if the user checks that box in the header template both the dropdown list with id of "ddlEndDate" and the textbox with ID of "tbEndDateYear" should be disabled or not be shown.
On page load the same controls in the item template should do the same thing.... So on page load if the box is check those fields are disabled or not visible. In the item template better to not be visible. If the user unchecks the box they should show... and then hide if checked again,
This needs to be done client side using jQuery 1.91.
The project should be completed in 48 hours.
We cannot give you a completed web part to work with but we can supply the ascx file and the code behind for it... the code behind is irrelevant in this case and can be commented out. We will take the solution and then integrate into our web part project. If the server side causes any issues you will need to help correct that.
The completed project can be the source VS files for an example web part if that is easier... .we can then integrate that into our project.
