ADF Email Form

We need a small lead generation form that outputs data in ADF - XML format, which is a specialized format for auto dealers. This allows that data to be parsed into our CRM system.

The lead generation form will be used on a specific site, but we would also like to iframe it into other micro sites we have and will require the ability to tweak the form so that the lead coming out reflects the site on which a lead was produced. i.e. We're a Ford dealership and the form will be used on an F-150 site but if we want to put that on a Ford Focus site we want to know where the leads were generated and need to change the data in the outgoing lead to reflect that.

The form will be iframed into a .net site with dimensions of approximately 350px wide.

The form needs to collect the following mandatory data:

*First Name

*Last Name

*Phone Number

*Email Address

This is our first post on Freelancer and we're not yet fully acquainted with the process but am happy to answer any questions and fill in the blanks if we've missed any details.
