Create a login form like the ones used by Kajabi

I currently use Digital Access Pass as my membership software. Their login forms are very simple and they look like this:

As you can see there is no design to them at all. But there is room to customize these forms as you can see here:

So I need someone to customize the login form using HTML and CSS to make it look like this:

[url removed, login to view]

But obviously it will have my own logo and colors to it.

I also want it to be easy for me to simply change the header picture and form background color should I wish to do so in the future. This should be a very simple task for someone with the necessary skills. I just need to ensure the Digital Access Pass form still works and logs users in and that I get the look I want similar to the [url removed, login to view] site.
