I looking for someone to develop me an AJAX form and a script to turn it into a printer friendly web page. There are a few notes to point out below.
- The printer friendly version must load in lightbox fashion
- The page will print to a letter sized shared printer
- 2 components of the form will be reliant of the choice made in previous results, where the AJAX comes into play.
Below are the required fields:
01. Operator code (drop down)
02. Customer account number (text box)
03. Customer name (text box)
04. Vehicle year (drop down) - list provided later
05. Vehicle (drop down) - list provided later
06. Colour code (text box)
07. Colour name (text box)
08. Quantity (radio buttons)
- Maxtub, Aerosol, 500ml, 1L, 2L, 3L, 4L, 5L
- If "Aerosol" display an extra quantity text box
- If "Aerosol" chosen, limit paint material options (see below)
09. Paint Material (radio buttons)
- 2K, 1K Celly, 1K Brushable, Solvent Basecoat, Water Basecoat
- If "Aerosol" chosen above, only Solvent Basecoat and Water Basecoat are enabled.
10. Extras (Checkboxes)
