Create pdf file for letterhead

I have lost the pdf file of my office letterhead, and I need someone recreate it for me so I can go to a printer and have the letterhead printed. I don't have a high resolution file of just my business logo. The best copy of my logo that I have right now is from a proof of a business card, which has the logo (see the attached file "logo card.") I would like someone to lift the logo (with the blue background) from that business card, and place it at the bottom right corner of the page, with the background color bled to the edge. The logo itself should be about 38 mm tall. Then, the background color of the logo should run to the right, so that there will be a horizontal blue bar at the bottom of the page. The blue bar should be about 43 mm tall. In the horizontal blue bar will be the addresses of my two offices, and my wed address (as seen in the file attached called "addresses.") I would like to have two versions of the letterhead: One with the logo and blue bar at the bottom of the page, and another version with the logo and blue bar at the top.
