Create Fantasy World Map

I'm in the process of writing a fantasy novel and I need to get a map for my world. It's a pretty small world, dominated by a couple mountain ranges in the north, with a large inactive volcano between them. Below the mountains are a couple forests, three rivers, two lakes, and a small town.

I have a model attached that I'd like a more professional map based off of. I have placeholder text for the names and can give those to you later.
