Complete web forms with data from PDF (20 sheets)

We have a web form which must be completed with data from an identical PDF.

We have 20 sheets with 340 checkbox. See attached image. The web form is identical to the PDF.

You have 8 hours to complete the work.

1. We provide an authentication id to the chosen freelancer.

2. The freelancer must indicate this id in the form and a number present in the PDF (Inscription id).

3. The freelancer must click on "Validate" button to begin. If "Freelancer id" and "Inscription id" are accurante, the following is displayed : "Freelancer and Inscription are available. You may answer the form."

4. The form can be completed. Freelancer don't have to save. Each answer is automatically saved in "realtime".
