Hello, I have a fairly simple project. I want to list a few things about a disease on a webform. Depending on whether a person checks 'yes' next to each parameter, i want to the website to suggest a plan of action. I already have the plan of action already written out for each combination of checkmarks. I want it to autodisplay the results at the bottom of the page based on the checkmarks. I want to the design of the form to be simple. I want it to display mainly for mobile phones, so let it be mobile friendly. I want it similar to this: Should be very simple for a person with knowledge of PHP, javascript, and Ajax if needed.

On line job booking system for a building maintenance company with approx 30 buildings to maintain. Each entry would go into a database along and Google Charts would be used to graphically display the number of entries filtered by type and date range. Data could be entered by using a series of 4 dropdown menus or manually into a text field. Below the text field, the text corresponding to the number would be displayed. See attached image for an example of the data entry form. Ideally this page would use the Bootstrap kit for styling and AJAX to reload the form after entry.
