Contest Brief

I need an excel form designed to look more professional. It filters results to users and they download the form, like attached.

I need to find a way to make excel look very professional with lots of contact options on it. If you can find a way to incorporate buttons into each row, that would be great. Want it to follow theme/brand of my logo.

Would like a summation of numbers listed on forms with potential graphs, layouts or charts charting: average PSF/month, average rent, etc

Keep in mind the code is written in PHP and NO HTML elements can be used.
