Contest Brief

I need a double sided flyer for my direct mail campaign for my Chiropractic office called Michigan Health Centers. I will upload the logo I want used and some office photos, as well as stock chiro photos, use them as you please. also I have 2 locations and would like a google map on the back side of the flyer for each location, phone number should be 989-895-5090, website is www.mihealthcenters.com. I need the flyer to be 8.5x11 in size. The offer is for an initial chiropractic treatment and is " NO OUT OF POCKET COST for the 1st visit" includes Consultation, Exam, X-ray(if needed) and Adjustment. I would like a few of the testimonials I have on Facebook as part of the flyer. That being said I want the flyer to not be cluttered. One of the final product formats should be an editable PDF so I can change the offer as needed. the first 4 pictures are from the essexville location the last 4 are from the saginaw location, you do not have to use all photos.

  I am not a graphic designer, I just know that I am able to edit pdf files on my computer, I am looking for simple so that I can edit the flyer when I need to for future advertising. That is what is important to me.... thanks

I want the logo with chiropractic only used for the flyer..... thank you. the pictures are their to be added to the flyer but I prefer some graphic work on the flyer also along with some of the photos, you certainly do not have to use all the photo or any.
