We are looking for someone to develop a simple wordpress plugin to control lab work sent externally. It needs to have option to Enter new entry / delete / modify / etc. The following fields are needed: Impression Taken (Date field) Dentist Name (free text field) Patient Name (free text field) Lab name (list from pre-populated list of 5 labs) ... this list can be hard coded. Notes (free text field) Collection Requested (date field) Impression Collected (date field) Job received (date field) Job cost (£ currency field) PCT booked (date field) Requerements: - Lab works will be listed as a table - Needs to have a modern looking UI - Once a new lab work is entered ... system needs to automatically send an email to the lab's email address request collection - Option to show/hide entries view based on any of the field For example: - Only show results with Collection requested between 01/Aug - 05/Aug - Only show results with Job received field empty - Only show results with Lab Name = ABC - etc - Possibility to export results on CSV format - Only wordpress registered users can access this plugin.
