I would like to hire a Microsoft Access Developer

I need an access database developed, with one form for data entry and two reports. The database will require the following fields; -Confined Space ID -Confined Space Location -Description of Space -Confined Space Determination - Is space enclosed or partially enclosed? (Y/N) - Is space intended or designed primarily for other than human occupancy? (Y/N) - Could the atmosphere have oxygen concentration outside the safe oxygen range? (Y/N) - Could the atmosphere have a concentration of airborne contaminant that may cause impairment, loss of consciousness or asphyxiation? (Y/N) - Could the atmosphere have a concentration of flammable airborne contaminant that may cause injury from fire or explosion? (Y/N) - Could the atmosphere have a stored free-flowing solid or a rising level of liquid that may cause suffocation or drowning? (Y/N) - Confirmed Confined Space as per AS/NZS2865-2009 Confined Space (Y/N) - Risk assessment conducted (Y/N) and have a link to a Word Document - Rescue Plan conducted (Y/N) and have a link to a Word Document - Access points to the space (number) and attach photos for each - Attach media/photos of space I would like the form to be able to enter all this information into the data base (auto date and time stamp. The reports I would want would be an overall of the whole data base and a single page on each entry
