I am looking to transpose an existing .docx template into a digital program for multiple users. As of now, we utilize an existing .docx template file as a guideline for creating reports. What I envision is being able to select drop down boxes for varying categories and create this form within a manageable program with admin rights to view pending and submitted reports. Example:

Included is a redacted report form for insight on what types of fields are set and do not change. We want to be able to have set responses to each questionnaire field. Each bold field is a category that has subset questions to be answered and filled out. A working knowledge of Jobnimbus and Companycam is a plus if integration is at all possible.
