SQL database dropdown menus and variables

The task involves creating 2 drop down lists and populating variables to be used with JavaScript from an existing sql database. Ideally the task would suit someone who is very familiar with sql databases, JavaScript and has an understanding of html. Please open provided excel and HTML templates to see what is required What is required? - 2 HTML file are required to be made. See notes in excel file. - 2 dropdown menus based on information contained in a sql database (supplied) - Some Information displayed depending on dropdown menu variable selected - Variables populated with data from sql database based on selected dropdown menus - Variables must be identical to column headings (see spreadsheet) - Variables must be able to be used with JavaScript What has been provided? - A HTML file to be used as a template. PHP database and table information located at top of page (password will be provided to freelancer). This also contains a list of all the variables to be imported - An excel spreadsheet showing the preferred layout - An excel file identical to the database file - Additional notes have been provided in the HTML file and excel spreadsheet Optional: - A button can be added or not, whatever is easiest - Any changes to the database need to be noted. An export of the final database will need to be supplied if changes are made Any questions let me know
