Modify a WordPress Plugin, Build an Form Processor

This project requires building an HTML form to process submissions on the front-end of WordPress. I need the ability to add multiple fields to the form as needed. Also, some drag and drop or checkbox functionality is needed. The plugin will then need modifications to save the submission into post meta fields. See the image attachment for more detail on the form and the changes needed. The plugin is also attached. The plugin was written by me and currently works to display and save the form data, but without the enhancements needed. It lets members create lesson plans, which are a custom post type. For the drag and drop/checkbox functionality, I need for the form to search through posts on my site for heading tags. I want to ability to select post headings or post titles to include in the lesson plan. This will let the teacher have a list of items that should be covered in the lesson. Requirements: - My site uses jquery, I don't want other libraries included - Deliver within 36 hours of accepting - Work can be done on my dummy site, FTP and WP login details will be given
