Looking for a web formbuilder w/database integration

Need a web form builder that will connect to multiple database types, but primary db will be MySQL. Also need to be able to use on a local machine stand alone, and either upload, or FTP coded forms to web site. Need administrative side to save forms as a reusable type control, for example a Name control may have properties of First Name, Middle Name, Last Name, or Title First Name, Middle Name, Last Name, Suffix, or First Name, Last Name, or Full Name...another example might be Address, with, or without visible labels with text field address, address 2, city, state/province, zip/postal, phone, email, notes, or text field, address, city, state/province, zip/postal, phone, email. NOTICE the second address has only one address field. Need to be able to place controls via HTML margin, padding, column, and row, along with div tags to group controls. Need regex, captcha, color, font, name, table, db, field, class, show/hide, access control (admin) options for controls. See image for complete description and details. There are open source projects, GNU, and GPL but none do all that I'm looking for. Build from scratch, or use open source and add, I don't care. Just so I know you have read this entire project, and looked at the attached image, and document, mention at the beginning of your post, "formbuilder on steroids". This can be a phase project where a milestone is paid as each phase is completed, and delivered. When you bid, bid correctly. I don't want to discuss the budget. Bid what you are willing to get paid for each phase of this project over the life of the project. I will not entertain bids higher than the maximum price I post unless you are willing to break it into phases for the completed project.
