I need a nice employment form for a web site that is functional via computer and mobile. I have attached a pdf that includes all the fields. It must have Captcha-like capabilities, and the Captcha word-image should change each time the form is accessed (or when someone refreshes the image).
The form results will be emailed. Also, the html page (around the form) needs to allow for alterations without complicated programming knowledge. So if perhaps you can embed the form inside an html doc so everything (but the form) can be altered, it would help a lot. That's just an idea, if you have a better solution, please share. Also, the page needs to allow the visitor to upload/attach a résumé from their computer. PS: I can send everything in a text file to make it easier to copy and paste, which should speed the process.
