Create simple web site orders php scripts

I need 4 really simple php scripts to drive my order system. Each script is defined in the attached document. I estimate the task at around 3 hours, the actual payment processing and account setup functions are NOT included in the scope of the project, they will be done separately. Please take your time responding, I will generally delete generic responses which arrive within seconds of me posting the project. The scripts have to be fully functioning, but I will upload them to my hosting account myself.
