I need some one to develop html code to get data from an online sql server table and populate dynamically in a drop-down menu where each menu values will be filtered based on the previous selection. Then the user has to input some fields, and all these data should go to another table on the sql when the user click save. Example: Drop-down menu no 1: country (from table_1) Drop-down menu no 2: city (from table_2) Drop-down menu no 3: street (from table_3) Date input field: input_1 Name input field: input_2 Button: save (to table_4) User will select the country, the when he selects the city from the drop-down list he should find the cities related to that country only, and when he select the city, he should see the street related to this city only, then he put date, name and click save. When user click save, country, city, street, input_1, and input_2 should be saved in table_4 I need a code to be inserted in a wordpress page where it is clear where to add the following: 1- Connection to data base: • Server • Username • Password • Database name 2- tables to populate in drop-down menu (table_1, table_2, and table_3). 3- Table to save the user data (table_4)
