Build a form for generating a PDF

Hi! We need someone who can develop a online generator (form) for our invoices. I've attached a pdf with comments for the fields we need. Please check it out (yellow markings). It is VERY important that the generated PDF is pixel-perfect. It need to look exactly like ours.
Some of the variables contains a calculation. Please check them carefully and let me know how many hours you approx. need to make this. If you finish this project properly, we will hire you for many other related things.
