Application Form Application iframe build in Existing Website

I need application form iframe in existing website to confirm participation on one event. Each person will receive personal password and with this password it will make registration to event. Database with all persons should be imported from Excel and we have 4 different categories of invited people (this means also 4 different options of confirmation, some people will have only access to hotel, some to hotel and dinner,...). After confirmation they should receive confirmation e-mail. I need also "administrator board" where I can see who has confimed participation,.. You receive from us all text and also sample of such solution. Work must be finished in next 48 hours.
