Create a Wordpress Template

We are changing themes on our existing Wordpress website.
The new website is running this theme

The old website has a custom template that allows users to register, create an account and input data. That page can be seen here...
[login to view URL] (the page lacks proper CSS but it is not viewable by the public)
I need the code from the above template ([login to view URL]) integrated with the code from the NEW theme [login to view URL] and a NEW template created.
I have tried simply uploading the old template to the new site, but that does not work, a new template is required. Please understand that I already know how to create a simple page template, but was unable to do so for this project, thus this post.

3. I need the code from [login to view URL] integrated correctly into [login to view URL] then a NEW TEMPLATE created.
