Create a PHP / JQuery Booking Form

I need simple PHP/jQuery form for website. Form should work similar as hotel booking forms where you select primary options, quantities and additional options are shown based on primary selection. Total amount must be calculated real time based on selction of options and extras. Form must be posted than with PHP to either email or file on server (will be decided later). Project must be functional, design is secondary importance. I expect script to plug it in runing website, replacing old static form. All additional information I'll give in chat.
