I'd use a reverse geocoding engine, most probably Google Maps API, to obtain the City/State/Country. Data-driven maps have been a recurring theme in my line of work, which is why I think I'm qualified to help you out with this project :) Map/place location can be easily done with openstreetmap/Google Maps, a task I've actually completed in the past, so I'll be able to base my work on that. Highcharts advanced lat/long maps is the perfect tool to display the kind of data you have, so doing that won't be much of a problem. Depending on wether you want this to be standalone or not, I could do a Wordpress Widget for easy integration or a standalone script, which can work on csv files or a database, if one is on hand.
