I need to create a single web page (pictured) which asks users three questions. Based on their responses, a URL is formed using parameters stored in the dropdown box. Each question is to be displayed once a valid selection has been made in the prior question. The values in the third box is cascades from the selection in the second. Once all three questions have been answered, the value from the 3rd box is passed as a parameter to the URL attached to the button at the bottom of the page. Clicking on this button directs the user to a SurveyMonkey page. The layout must be very simple and clear. I will provide the spreadsheet of values for the drop-down boxes, plus image files for the logo.
