I need a mobile responsive lead capture system built on a LAMP Stack. This is the requirements for the project. The initial pages will use a sliding transition similar to trulia [login to view URL]|global_nav I will provide a branch chain list of questions (see attached for example). This will move the user through a process that will take them to the first page of a form. We will be using hidden fields on the first page of the form to categorize the user for our database. The first page of the form will instruct them to set up a login and ask for their Name & Email address and password. Alternatively they can choose to login with their Google or Facebook accounts (if this method is chosen we collect their name and email). Information is saved immediately into our database and they are logged into the system where they are asked to complete a multi-step form. Role will be set to applicant. Applicants can only see their personal information submitted in form. They will have the ability to save and return later and also to come back and edit answers already submitted if needed. This needs to be integrated with Sendy API [login to view URL] so that name and email are assigned to the list that matches the hidden on the first page of the form. ADMIN REQUIREMENTS. Need super admin and admin role. Super admin has all admin functionality plus the ability to add additional admins. Admin needs a login that will allow them to view a list of applicants. List will include name, email, phone, date submitted,Type (hidden field) and status. Status for applicants who completed the application is automatically set to "Ready" Status for applicants who have not completed the application is set to "Incomplete" and automatically changed to "Ready" once completed. When an applicant is clicked on this will open up a new page with all the applicants information. Admin can update status to viewed, Contacted, Awaiting Response, Scheduled, Interviewed, On Hold, Not a Match and Complete" All Complete and not a match applicants will be removed from active list and placed into an archive. Notes are required for applicants marked not a match. Admin needs the ability to send an email to the applicant through the system. This email is tracked in the notes section which shows a history of all email communications. When applicants reply to an email that is added to the history as well. Admin also has the ability to add notes the the history. All histories must be date and time stamped. Admin needs ability to sort list of applicants by name, date and type. They also need the ability to search for specific applicants by name, date, email, status and phone. Admin needs ability to export data into csv file. Admin should choose which data fields need to be exported. and can segment exports by date, status or type.
