Build a Simple Website with Database (Like Simple Member System)

I need a private website created, it will act like a match-making system. These are the requirements: 1. Public front-end accessible by a domain URL - A registration form that have several fields, such as Full Name, ID, Address, Phone Number, Age, Gender, Biography, Picture - Once click Next, proceed into a preview page with all the details, option to Edit or Submit. - Once submitted, display a thank you message. - End 2. Back-End - Admin can login the database - Admin can view all submitted entries in a list, with all info submitted by the public, and also Date submitted. - Admin can Add, Edit or Remove any Entry 3. Front-End (Private, Accessible by only an admin account, used to show the member who walked into the office) - Catalogue of all the entries, Grid and List view of all the entries - Display only Full Name, Gender, Biography and Profile Picture. - Once choose an entry, Admin will come over to login and then can display all the information like Telephone, Address and so on. So the one who browse the catalogue can not view all the information but only the 4 fields as mentioned above. Design is not the utmost important thing, but it has be pleasant and modern, not those outdated design. You will need to come out with the concept/mock-up before proceed with the project, please go through all the requirements before you bid on this project, I will not repeat the requirements in our discussion, but feel free to ask any questions.
